Alfatradiol (17α-estradiol) is a weak estrogen stereoisomer that inhibits 5-alpha reductase locally in the scalp. Unlike 17β-estradiol, it has minimal systemic estrogenic activity, making it suitable for both sexes. It reduces local DHT conversion without affecting serum hormone levels.
